Chorus Music Academy Inc. Summer Camp Terms and Policies

Chorus Music Academy Inc. (“we”, “us”, “school”, “Chorus”, or “Chorus Music”) provide in-class and online instrumental, vocal and theory lessons. Other services include song writing, recording, original music production, recitals, camps, exams, and competition preparation. The following terms and policies apply to Chorus Music’s Summer Camp Programs.
- Pizza Lunch: Included on Friday (cheese or pepperoni). Please let us know which option you would like during registration.
- Meals: Campers will be required to bring a lunch and 2 snacks per day. We are a nut-free facility. Please do not send any nuts, fish, or shell-fish items. We recommend labeling lunch boxes and personal belongings.
- Water: Campers are required to bring a water bottle, ideally labelled.
- Extended Hours: 8:00 AM - 5:00 PM at $65+HST/week per camper.

Full payment required with registration by e-transfer, credit card, debit card, or cash. - Refunds and Cancellations: A full refund will be issued for cancellations made by April 1st, 2025. After this date, until 10 days prior to the start of the camp, a $50 administrative fee per camper will apply for cancellations and a credit will be issued for the remaining balance. The credit may be used for a different 2025 summer camp or for 2025 summer lessons. Cancellations made within 10 days of the start of the camp can only be made with a doctor’s note and will be eligible for a credit with a $50 administration fee. No refund will be issued for cancellations made within 10 days without a doctor’s note, and a $50 administration fee per camper will still apply.
- Cancellations by Chorus Music: Chorus Music reserves the right to cancel summer camps due to low enrolment by April 1st, 2025. A full refund will be provided if participants cannot switch to a different program.
- Conduct: Participants are expected to behave appropriately and responsibly during summer camps. Chorus Music Academy fosters an inclusive and supportive environment where bullying, harassment, or inappropriate behavior will not be tolerated. Consistently disruptive or inappropriate behavior by participants towards other campers, staff, or the school could result in the participant being removed from the program with no refund.
- Allergies and Medical Conditions: It is the responsibility of the parent/guardian to notify Chorus Music Academy Inc. about any allergies, medical conditions, or medications participants have or may require, and to provide the appropriate medications or remedies to Chorus Music Academy Inc. and to ensure their child(ren) are carrying the appropriate medications or remedies they require. While Chorus Music Academy Inc. makes its best effort to uphold a nut-free facility and keep our environment safe from common allergens, we are not responsible for and cannot guarantee that an allergen will not be brought into the facility. We will be monitoring snack breaks and lunch but are not responsible if a camper with an allergy shares food with another camper or has a reaction. It is the full responsibility of the parent/guardian to ensure their child(ren) understands that they are not to share food.
- Emergency Contact: Parents/guardians must provide emergency contact information at the time of registration. In case of an emergency, this information will be used to contact the designated emergency contact if the primary guardian is unavailable.
- Late Pickups: If you have not purchased extended hours, our daily extended hour rate will be charged for late pickups after camp hours. A fee of $10 per 15-minute interval will be charged for late pickups beyond extended hours.
- Internet Use and Personal Electronics: It is the parent/guardian’s responsibility to ensure their child(ren) use their own personal devices in a responsible, legal manner. Chorus Music is not responsible for monitoring personal electronic device usage by campers and are not responsible for any actions taken by campers on their devices or Chorus Music devices.
- Parking: Parking is available all around the building excluding reserved spots that are not available to us at any time.
- Liability Waiver: Parents/guardians are responsible for the well-being and safety of their children at all times. This includes drop-off and pick-up of their children on the premises or at offsite events that are hosted or sponsored by Chorus Music. Chorus Music Academy Inc., including its directors, officers, employees, and agents, shall not be liable for any injury, illness, allergic reaction, accident, death, loss, theft, damage or any other harm whatsoever which may be sustained by a student, parent, guardian, or guest at our facility, or at an offsite event hosted or sponsored by Chorus. You waive any claims, demands, and liabilities against Chorus arising from your participation in Chorus programs and events/activities, including participation by you and/or your child(ren) and guests.

Photo and Video release: By registering with Chorus, you grant Chorus
permission to take photographs and audio/video recordings of you and/or your child(ren)’s
participation (“Content) in Chorus programs/events/activities. You also grant Chorus
permission to use, reuse, modify, publish, and republish the Content in any format or media,
including print and online, for promotional purposes, in perpetuity. Chorus will use the
Content without compensation to you, your children, and your guests. You waive any rights to
prior inspection and approval of the Content and its use. You waive any claims, demands, and
liabilities against Chorus for capturing and using the Content. To opt out of this term,

You need JavaScript enabled to view it. . - Dispute Resolution: If a dispute arises out of and/or in connection with the terms and policies of this document, the parties agree to meet to pursue resolution through negotiation/mediation or other appropriate dispute resolution process before resorting to litigation.
- Governed by the laws of Ontario: This agreement constitutes the entire understanding between the parties and supersedes all prior and other representations, understandings, or agreements between them, written or oral, respecting the subject matter herein. This agreement may be amended only in writing, shall be read without regard to section headings, shall be governed by and constructed in accordance with the laws of Ontario.
- Duration: The terms and policies on this form are valid until replaced by a new camp form.
